Service Code 024: Person-Centered Planning (PCP) Services Empowering Choice, Independence, and Self-Direction through Meaningful Planning At My Life My Way SDS, our Person-Centered Planning (PCP) services form the foundation of individualized supports across all programs, including Supported Living Services (SLS) and Independent Living Services (ILS). Each planning process is designed to help individuals identify their strengths, preferences, and life goals while promoting self-determination, inclusion, and personal responsibility. Using the principles of Title 17, the Home and Community-Based Services (HCBS) Final Rule, and DDS Person-Centered Practices, our facilitators work directly with participants, their families, and support teams to create plans that reflect each person’s vision for a meaningful life. These plans inform how SLS and ILS supports are delivered—ensuring services are tailored to promote independence at home, successful community engagement, and ongoing skill development. Through open dialogue and collaborative discovery, every individual is empowered to take an active role in defining what “a good life” looks like for them. Each plan becomes a living roadmap that guides service delivery, supports accountability, and celebrates progress along the way.

Self Determination Ongoing Supports
(Service Code 099)

At My Life My Way SDS, Ongoing Supports (099) provide individualized, person-centered assistance that helps participants maintain progress toward their goals and adapt as their needs evolve. Guided by Title 17, HCBS Final Rule, and DDS regulations, supports are flexible, choice-driven, and built around the individual’s preferences. These services complement Person-Centered Planning (024), ensuring continuity, independence, and self-direction in how each person lives, works, and engages within their community.

Resource Development

Resource Development at My Life My Way SDS focuses on connecting participants with community-based supports that enhance independence and inclusion. We identify, coordinate, and develop resources tailored to each individual’s goals—ranging from housing and employment to social and recreational opportunities. Our approach ensures every person has access to meaningful options that promote growth, choice, and long-term stability.

Through our social-recreation programs, we prioritize socialization as a vital part of personal growth—creating meaningful opportunities for connection, inclusion, and active participation in community life.

Building Life Skills and Independence

Our services are designed to help people develop life skills, build relationships, and achieve personal goals—supporting them in living life their way.” Building Life Skills and Independence for a Stronger Future At My Life My Way SDS, we focus on developing essential life skills that empower individuals to live more independently and confidently. Through personalized supports and community engagement, participants build the tools and habits needed to create a self-directed, fulfilling future.

Our social recreational activities provide individuals with developmental disabilities the opportunity to build friendships, explore new interests, and engage in meaningful community experiences. Activities may include art and music classes, group outings to parks, museums, or sporting events, fitness and wellness programs, and fun gatherings like game nights or holiday celebrations. These opportunities not only encourage creativity and enjoyment but also promote inclusion, social skill development, and a sense of belonging—helping each person participate fully in the life of their community while enjoying leisure in ways that reflect their personal interests and goals.

Our Independent Facilitator supports empower individuals to take charge of their own lives by guiding them through person-centered planning and helping them identify goals, preferences, and resources. We provide personalized assistance in accessing services, building community connections, and ensuring that every decision reflects the individual’s voice and vision for their future.

Self-Determination Supports are designed to empower individuals with developmental disabilities by providing personalized guidance every step of the way. This includes assisting with enrollment to ensure individuals and families understand their options and rights, offering orientation to introduce them to available resources and the planning process, and delivering ongoing support to help them achieve their goals. Through consistent advocacy, coaching, and coordination, we make sure each person is equipped with the tools, confidence, and assistance they need to direct their own services, build meaningful community connections, and live with dignity, independence, and choice.

 
 

Comprehensive Supports for Independence and Inclusion

Our DSPs, Independent Facilitators, and ILS/SLS Background ensures supports work together to promote choice, independence, inclusion, and meaningful social recreation for individuals with developmental disabilities.

Supported Living Services (SLS) and Independent Living Services (ILS) are designed to help individuals with developmental disabilities live safely, confidently, and successfully in their own homes and communities. SLS provides personalized, around-the-clock support tailored to each person’s needs—ranging from assistance with daily living skills, health and safety, to community integration. ILS focuses on teaching and strengthening skills such as budgeting, cooking, personal care, and navigating local resources, empowering individuals to increase independence and self-reliance. Together, SLS and ILS foster dignity, choice, and inclusion by ensuring that people have the support they need to thrive in the setting of their choice.
