Person-Centered Planning and On-Going Supports

At My Life My Way SDS, 024 Person-Centered Planning and 099 Ongoing Supports work together to help each individual lead a self-directed, meaningful life. PCP identifies a person’s goals, preferences, and strengths, while 099 provides the ongoing guidance and coordination needed to turn those goals into action. Our approach follows DDS, Title 17, ABA, and HCBS Final Rule standards—ensuring services remain person-led, compliant, and empowering. By combining planning with consistent follow-up, we help individuals build independence, self-advocacy, and lasting community connections.

Community Living Support

Community Living Supports help individuals with developmental disabilities live safely, independently, and fully engaged in their homes and neighborhoods. These supports are tailored to each person’s needs and may include assistance with daily living skills, budgeting, meal preparation, transportation, and building social connections. At My Life My Way SDS we focus on empowering individuals to take an active role in their lives while remaining connected to the people, places, and activities that matter most to them. Through respectful, person-centered support, we help create a stable foundation for individuals to thrive within their communities.

Independent Facilitators

Independent Facilitator Services Offered by My Life My Way SDS At My Life My Way, our Independent Facilitator (IF) services are designed to empower participants in the Self-Determination Program by placing their voice, goals, and choices at the center of the planning process. Chosen and directed by the participant, the Independent Facilitator is a trusted support—not involved in providing other services—who helps guide and implement the participant’s Individual Program Plan (IPP). Our Independent Facilitator services may include: Actively participating in the person-centered planning process. Helping the participant identify both immediate and long-term needs, preferences, goals, and objectives. Assisting the participant in making informed decisions about their individual budget. Exploring and developing service options that meet the participant’s goals and accessing community-based supports. Advocating on behalf of the participant throughout the planning process and in securing the necessary services and supports. Participants and families working with My Life My Way SDS can choose to hire or contract with us for Independent Facilitator services. The specific responsibilities will be clearly outlined in the IPP. Participants may also opt to use their regional center service coordinator as their facilitator; however, Independent Facilitator services from My Life My Way SDS do not duplicate the role of the service coordinator. We are here to ensure that each participant’s plan reflects their unique vision, values, and direction—because every life deserves to be lived your way.

Direct Support Professional (DSP)
At My Life My Way SDS, our Direct Support Professionals are more than caregivers—they are partners in empowerment. Guided by our philosophy of person-centered, strengths-based support, DSPs work alongside individuals with developmental disabilities to promote independence, self-determination, and meaningful community inclusion.

Our DSPs provide compassionate, respectful, and individualized assistance in all areas of daily living, including personal care, meal preparation, transportation, skill development, and community participation. They are trained to recognize and build upon each individual’s unique abilities, encouraging self-reliance while ensuring safety, dignity, and quality of life.

Drawing on our team’s extensive experience in Supported Living Services (SLS) and Independent Living Services (ILS), DSPs collaborate with each person, their family, and their support network to create an environment where goals are not just met—they are exceeded. They foster opportunities for social connection, learning, and recreation, ensuring that every person has the tools and encouragement to live life their way.

Behavior Technician – As Needed

My Life My Way SDS

At My Life My Way SDS, our As-Needed Behavior Technicians (BTs) deliver flexible, person-centered support consistent with California Title 17 regulations, Home and Community-Based Services (HCBS) Final Rule standards, and all Board Certified Behavior Analyst (BCBA) guidelines. Each BT operates under the clinical supervision of a BCBA or Behavior Specialist, implementing individualized Behavior Support Plans (BSPs) that promote autonomy, skill development, and inclusion for individuals with developmental disabilities.

Key Responsibilities

  • Implement Evidence-Based Interventions:
    Apply teaching strategies and positive behavior-support methods as prescribed by the supervising BCBA, ensuring all procedures comply with BACB ethical codes and DDS guidelines.

  • Promote Dignity and Choice:
    Provide compassionate, person-centered assistance in home, community, and program settings, supporting participants to engage meaningfully in their environments.

  • Collect and Maintain Accurate Data:
    Record objective behavioral data and session notes consistent with Title 17 documentation standards to guide data-driven decisions and treatment fidelity.

  • Ensure Health, Safety, and Rights Protection:
    Uphold all participant rights, confidentiality, and mandated-reporter obligations in accordance with DDS, IRC, and HCBS policy.

  • Collaborate Within an Interdisciplinary Team:
    Partner with the participant, their family, BCBA, and other providers to ensure consistency, transparency, and accountability across all supports.

Position Overview

Because this is an as-needed role, flexibility and adherence to professional standards are essential. Technicians may be called to fill staff absences, provide transitional or short-term supports, or assist with community-integration events.
